Deep Plane Face Lift with Deep Neck Lift - Patient 1000

This 60 year old lady described her issue as a loss of facial volume coupled with the development of jowls and loss of skin quality (which she characterised as becoming more crepey). She had previously had filler to her nasolabial folds and cheeks. The surgical plan aimed to address her under-projected chin, oblique neck (dewlap), jaw line (sagging of her jowls), nasolabial folds and marionette lines, skin wrinkles and facial volume loss.

She underwent a deep plane face and neck lift together with modification of the submandibular glands and digastric muscles. Her deep neck fat (that would normally be removed) was used as a an autolgous chin implant to improve her chin projection. Facial volume was restored using micro fat graft from the medial aspect of the knees. She is seen in these images at 1 year following surgery.

This patient requested facial rejuvenation surgery stating that she had always considered herself to be facially you however over the previous 12 months she had noticed a significant change in the shape of both her face and neck (the changes having become more obviously to her through the increased use of video calls with her work). The most obvious issue to deal with was her oblique neck which had significant fatty deposits within it.

The surgical plan aimed to reduce the volume of the neck and to soften the marionette lines and nasolabial folds by lifting the deep facial tissue (which would also improve her sagging jaw line). She underwent a deep plane face and neck lift with submandibualr gland modification and digastric muscle plication. The post operative images are 3 months following surgery.

This 56 year patient attended clinic with several goals. She stated that she had always had a full neck and receding chin and that since losing a significant amount of weight had developed significant slackness of her facial skin. Her primary aim was to achieve a more defined jaw line. The surgical plan was to improve neck definition and raise the position of her low lying hyoid bone, augment her chin, restore facial volume and address the jowls and facial skin folds.

She underwent a deep plane face and neck lift together with modification of the submandibular glands and digastric muscles. Her deep neck fat (that would normally be removed) was used as a an autolgous chin implant to improve her chin projection. Deep Plane Face Lift - Patient 1003

This 65 year old lady sought my help stating that she felt that she looked ‘tired and haggered’ and that she thought that her face looked asymmetric (and that previous dental extractions had made the situation worse – which it would have done). She did not want any surgery to her neck. The surgical plan was 3 fold – address the drooping of the cheeks and jowls (which had resulted in deepening of the marionette lines, nasolabial folds and the development of jowls), to correct the asymmetric appearance of the mouth, marionette lines and upper lip, and to restore facial volume.

The issues to do with the marionette lines and upper lip were addressed with a gliding marionette lift (which can be considered an extended for of lip lift) whilst a deep plane face lift was used to lift the facial structure. Facial volume was restored using microfat grafting. She is seen in the photographs at 3 months post operatively and then again at 2 years (where you can see the effects of the surgery really hold up over time) Dave- this is if you want to add in the second set but I don’t have these for many patients so I’ll let you make the call on this

Deep Plane Face Lift with Deep Neck Lift - Patient 1004

This 58 year old patient attended clinic looking for a solution to the way in which her facial appearance had changed. She described feeling as though her face and neck had dropped, that she had lost her jaw line, developed jowling together with banding on both sides of her neck. Her sister had had a a facelift which made the changes seem more obvious.

The surgical plan focused on reducing the volume in her neck (by removing fat and reducing the size of her submandibular glands) together with restoring volume tot he midface and tightening the jawline. The deep plane facelift approach was used to treat the jaw line and mid face regions together with micro fat grafting (for volume restoration). The neck issues were addressed with a deep neck lift. The pictures show her before and at 6 months after surgery.

This 60 year old lady had been recommended to come and see me when she had mentioned that she was considering facial rejuvenation surgery. Her main concerns were that she felt her neck had aged significantly and that she needed the lower part of her face lifting (she stated that she felt very self conscious). Additionally she had a big family event coming up which she wanted to look a million dollars for (and hence was seeking advice about facelift surgery). From a surgical point of view she had undergone marked soft tissue volume loss as well as developing skin slackness through the lower face and neck.

In the latter area her platysma muscle had formed visible bands on both side of her neck. The surgical plan was to tighten the soft tissues and replace the lost volume. A deeplane facelift was used to tighten the sagging jowls and cheeks, a deep neck lift was used to repair the platysma and microfat grafting was employed to restore volume to her face. She is seen here in the post operative images at 4 months following surgery.

This 54 year old patient came to see me as she felt that she was beginning to look like her mother. She stated that she had noticed a change in the shape of her neck and additionally felt that she was losing her jaw line. A deep plane facelift was used to tighten her jaw line together with micro fat grafting to restore facial volume.

To tighten her neck and improve the angle formed between the front of her wind pipe (trachea) and the tip of her chin (known as the cervicomental angle) a deep neck lift involving submandibular gland reduction and a corset platysmaplasty was performed). She is seen her a year following on form her procedure.

This 52 year old patient came to see me as she was concerned about the shape of her neck and the development of marionette lines. She felt that she no longer recognised herself in the mirror.

The treatment plan aimed to reduce the volume in her neck (subplatysmal fat removal, digastric muscle reduction and submandibular gland reduction), remove her buccal pad pads (which were contributing to her marionette region fullness) and elevate the facial soft tissues with a deep plane facelift. This 69 year old lady came to see me due the changes she had become aware of in her lower face and neck. This included platysmal banding in the neck, heavy jowls blunting the jaw line and deepening of the marionette lines. We undertook a deep plane facelift together with a deep neck lift (with submandibular gland reduction) and a gliding marionette lift to address these issues.

The post operative images shown here, taken one year following surgery demonstrate improvement in all the areas of concern, providing a rejuvenated appearance without changing the overall character of her face.

This 60 year old lady attended clinic regarding the shape of her neck (as her family had a tendency towards developing a heavy neck). She had noticed her neck had started to lose its shape and that this was becoming more accentuated as time passed. Her aim form surgery was to restore a more defined appearance to her neck. We undertook a combined plane facelift and neck lift to define the neck and jaw line region. As can be seen in the post operative photographs taken at 3 months following surgery, the combined approach of addressing the deep structures of the neck together with elevating the lower face has provided an improvement in both jaw line definition and neck shape (the aims of the procedure).

This 58 year old patient came to see me in clinic with general concerns about her facial appearance. An accomplished athlete, she felt her face and neck appeared older than how she felt and how her body looked. We undertook a deep plane facelift and neck lift (with submandibular gland reduction) together with facial fat grafting to restore volume to midface.

The post operative images shown here are 2 years following surgery and demonstrate how the combination of volumisation and elevation of the face provide a naturally rejuvenated appearance whilst the neck lift (and lower facial elevation) have provided a significant improvement in jaw definition and neck shape.

This 54 year old lady had concerns about the she of her neck (which she described as heavy and that ran in the family). Additionally she felt that she had begun to develop jowling and that all of these features made her feel very self conscious.

A deep neck lift which included resection of subplatysmal fat, digastric plication and submandibular gland modification was combined with a deep plane facelift to improve the contours of the neck and jawline whilst fat grafting was used to improve mid facial volume and soften the nasolabial folds.

This 67 year old lady attended clinic looking to improve the shape of her jaw line and also produce a more defined angle between the front of the neck and chin. She underwent a deep plane face and neck lift together with fat grafting to restore facial volume. Her submandibular glands were reduced as part of the procedure to help define the submandibular region.

Having previously undergone a face and neck lift elsewhere 8 years previously, she was already looking younger than her chronological age but the additional intervention achieved a softening of overall facial features and defined her areas of concern without producing a stretched or worked on appearance (which is always a risk with secondary procedures).

This 66 year old lady sought advice over changes to her facial appearance that she felt made her look ‘grumpy, sad and negative’ (which was definitely not in keeping with her personality). She underwent a deep plane facelift and neck lift (with submandibular gland reduction) together with facial fat grafting and a gliding marionette lift.

The post operative photographs seen here are taken at 6 months following surgery and show a softening of her facial features together with a restoration of volume to the affected regions of the face. The jaw line and neck definition are also improved however there is no hint of the stretched appearance that is frequently seen with face lift surgery in such cases..

This 60 year old patient attended clinic with concerns about the shape of her neck and deepening of her marionette lines and nasolabial folds. Additionally, she felt that her upper lip had lengthened significantly and wanted this addressing as part of the panned surgery. She underwent a deep plane face and neck lift (during which submandibular gland reduction was performed together with sub platysmal fat removal and digastric muscle plication) together with fat grafting and an extended gliding marionette lift (within which a lip lift was incorporated).

The post operative photographs here are taken at 9 months following surgery and demonstrate a significant change in all the areas of concern without changing the essence of her facial appearance.

This 65 year old lady attended clinic concerned about loss of shape in her neck and the development of jowls (together with an overall feeling of looseness in her facial skin resulting in the development of marionette lines). She had a significant amount of facial volume loss and fat grafting was used to improve mid facial volume in conjunction with a deep plane facelift, neck lift (with submandibular gland reduction, digastric plication and the usual corset platysmaplasty).

Deep Plane Face Lift with Deep Neck Lift - Patient 1016

This 62 year old lady requested surgery to help tighten her jaw line, elevate her midface and improve her neck definition. Fat grafting to her midface was performed (in the form of both microfat injections and solid structural grafts placed next to the bone overlying the cheekbone area) to increase midfacial volume. This was combined with a neck lift and submandibular gland reduction to reduce fullness in the area beneath the jaw and a deep plane facelift to elevate the cheeks and jaw line.

The post operative photographs shown are taken at a year following surgery and demonstrate how a reduction in submental volume combined with cheek elevation produces a sharply defined jaw line and rejuvenated appearance without the signs usually associated with traditional facelift approach.

Her aims from surgery were to appear fresher and have a natural non operated look following surgery. The combination approach of a deep plane facelift with fat grafting for volume restoration and a neck lift (including submandibular gland reduction, digastric plication and the usual corset platysmaplasty) were performed.

The post operative photographs seen here are taken 8 months following surgery and demonstrate a refreshed appearance to the face and neck without an operated on look (ie; a natural result).
